How Inflammation in the Choroid Plexus May Be Driving Long COVID’s Most Debilitating Symptom
I’ve spent years trying to understand the enigma of long COVID—why it lingers, how it shifts, and most of all, what’s driving the debilitating brain fog that so many patients describe.
Now, I believe we may be looking at a breakthrough.
The focus? A tiny but powerful structure deep within the brain called the choroid plexus. It’s not a household term, even in medical circles, but it may be central to one of long COVID’s most persistent symptoms: cognitive dysfunction.
The choroid plexus produces cerebrospinal fluid and also serves as a key site for immune surveillance—deciding what gets into the brain and what stays out. Almost like the lymph node of the brain. But what if it becomes inflamed?
In recent analyses, including imaging studies and histological reviews, we’re seeing signs that the choroid plexus—particularly in the fourth ventricle—is under immune attack in long COVID. This region sits right next to the brainstem, which controls wakefulness, attention, and autonomic balance. Inflammation here could disrupt communication between neuronal immune cells, impair the clearance of inflammatory molecules, and even contribute to astrocyte swelling—precisely the conditions that might feel like brain fog.
And here’s the most fascinating part: this pattern of dysfunction resembles the early effects of alcohol intoxication. The same coordination issues, memory lapses, and slowed processing. But in long COVID, the “buzz” never fades—it becomes the new normal.

We may be looking at a self-perpetuating cycle: microglial activation triggering cytokine storms in the choroid plexus, impairing its barrier function, and allowing inflammatory signals to loop back into the brain. It’s a silent storm—but now, we may know where to look.
Understanding this gives us a new direction for interventions. If we can calm the inflammation at the choroid plexus, we may finally break the feedback loop and start restoring clarity.
This is just the beginning of what I believe will reshape how we treat post-viral syndromes. The choroid plexus may hold the key to unlocking recovery.
