One interesting difference between Long Covid and ME/CFS (in many respects, overlapping conditions) is that there does, from numerous accounts in the patient communities, seem to be a slow or very slow tendency to improvement with time in Long Covid symptoms, whereas ME/CFS folk can remain very stuck for years, even decades. The reason for that difference may also prove significant.
Dr. McMillan, The reason that Long COVID symptoms are not improving in many patients is that most providers are treating the symptoms and not the cause of the symptoms. Mitochondrial dysfunction (MD) is demonstrably present in most LC patients per relevant published literature. Mitochondrial vitality is central both to innate immune response to viral infections and of course, to production of ATP. MD is treatable with appropriate care for the mitochondria and by altering diet and lifestyle choices to minimize inflammation.
